CONDITIONS OF ENTRY:

1. Information on how to enter and prizes forms part of these conditions of entry.

2. Entry is open to children aged up to 10 who are residents of Australia and who receive an official entry form from a participating pharmacy.

Employees and their immediate families of the Promoter and its agencies associated with this promotion are ineligible.

3. Promotion commences at Midnight EST 1st January 2009.
Entries close on 5th March 2009. Only entries completed on the official entry form will be accepted. This is a game of skill and chance plays no part in determining the winner. Each entry will be individually judged based on creative merit. Judges decision is final and no correspondence will be entered into.

4. Only one entry per child permitted.

5. The best entry as determined by the judges will win a $200 Toys R Us Gift Voucher. The next best 4 entries as determined by the judges will each win a $50 Toys R Us Gift
Voucher and a pack of Little Squirts Probiotic 30 Chewable Tablets (Total value of $65.15).

6. Prize is not transferable or exchangeable and cannot be
taken as cash. No responsibility is accepted for any
variation in the value of the prize.

7.No responsibility is accepted for late, lost or misdirected entries.

8. Winner will be notified by telephone.

9. The Promoter shall not be liable for any loss or damage
whatsoever which is suffered (including but not limited to indirect or consequential loss) of personal injury which is suffered or sustained as a result of taking any of the prizes, except for any liability which cannot be excluded by law.

10. Prize will be awarded to the winner’s parent or guardian.

11. The participating pharmacy collects entrant’s personal
information in order to conduct the promotion. If the
information requested is not provided, the entrant may not participate in the promotion. The personal information will only be used for the purpose of conducting the promotion. Once the promotion has been conducted the personal information will be destroyed, save for the publication of the winner's name on its website or promotional material. To
request access, update or correct any information direct enquiries to the pharmacy where you have entered this competition.

Newby with a question about this comp. It says you have to have the official form from the pharmacy. It the downloaded entry ok?
