Open to residents of Australia. To enter, consumers must complete an entry form. An entry form may be left in a competition box (for pick up), provided to our promotions people, given to a consultant or forwarded to our office at Level 4, 41 Currie St, Adelaide SA 5000.
I found this at McDonalds at Darlington SA, not sure where else it is available. If anyone knows where more entry boxes are, please post them on here.
