Travel insurance advice

I am hoping that some of the experienced lottos travelers could advise me about travel insurance.
We are planning an overseas holiday soon and are looking for travel insurance. Hubby has several diagnosed medical conditions. Whilst his are on the 'minor end' of the scale and is under 60 a lot of travel insurance companies seem to exclude them as he has some of the 'multiple' listed ones. ie) type 2 diabeties, combined with moderately high cholesterol & slightly high blood pressure (silly genetics). All are well controlled with minimal medication.
The cover that we mostly require is medical - with the option to be transported to Australia in the event of the 'worst case senario'.

Is anyone able to suggest providers that they have used that cover these conditions.
